footnote crash

### Describe the bug

When running mdformat on the following Markdown, it crashes.
```markdown
[^a]: Lorem. [^b]

[^b]: Ipsum.

A [^b]
```
Not saying the above makes a lot of sense, but mdformat should probably not fail hard.
